In social work, documentation isn’t just a task; it’s an ethical act.


The NASW Code of Ethics reminds us that clear, accurate, and respectful records are essential to good practice. 


As an example: 


 📘 Standard 3.04 (Client Records) emphasizes the importance of documenting services in ways that support continuity of care and uphold client dignity and confidentiality.


But ethical questions often arise:


 • What details truly belong in the record?


 • How do I balance transparency with discretion?


 • What’s legally required vs. clinically relevant?
